Run the four-question screen
- Is the patient unstable or is there an immediate threat?

Build a one-line representation
Compress the vignette into a short clinical sentence using discriminating features. This reduces distraction and helps retrieve the right illness script.
Do not repeat every fact. Include what changes probability or urgency: tempo, age, setting, exposures, key positives, and meaningful negatives.
Respect sequence
A correct test can still be the wrong next step if stabilization comes first. A definitive treatment can be wrong before confirmation when the patient is stable. Ask what the safest appropriate action is at this exact moment.
Use the runner-up test
After choosing, identify the most tempting alternative and state what would need to change for it to become correct. This builds contrastive reasoning that transfers to future vignettes.
